Forms
Bypass Report Form
The reporting form for Sanitary Sewer Overflow (SSO) or Wastewater Bypass. In the event of any sewer overflow, regardless of size, operators should verbally notify MDEQ within 24 hours, and submit a completed bypass report within 5 days.
Duly Authorized Representative Delegation Form
Authorized Signatories of NPDES permits can submit this form in order to delegate submittal authority to a Duly Authorized Representative.
