To make application for certification as a wastewater operator, select the appropriate form below. Forms are in Adobe format. You may complete the Application forms on screen then print and forward with the required attachments to:

If you are applying for reciprocity, you MUST provide the “New Certifcation” application and the “Reciprocity” application plus the necessary number of letters of reference.
